Woman Killed in Highway 30 Head-On Crash Near Scappoose as Oregon State Police Investigate Collision That Left Another Driver Seriously Injured

Posted on October 24, 2026

SCAPPOOSE, OR — A woman was killed Monday evening in a head-on crash on Highway 30 south of Scappoose, according to Oregon State Police. The collision happened around 6 p.m. on Oct. 19 and involved a Subaru Outback and a Volkswagen Bug.

Police said the Subaru was traveling westbound when it crossed into the opposite lanes and struck the Volkswagen driven by 57-year-old Rachel Ann Mitchell. Mitchell died at the scene. Authorities did not release her city of residence.

The other driver was taken to a hospital with serious injuries. First responders initially reported that two people were critically injured, but Oregon State Police later said one person died and the other survived with serious injuries.

Investigators say the Subaru crossed into oncoming traffic

Oregon State Police said the crash began when the Subaru Outback moved out of its westbound lane and entered the opposing lanes of traffic. That movement led directly to the head-on impact with Mitchell’s Volkswagen Bug.

Officials have not said what caused the Subaru to drift across the highway. The crash remains under investigation, and no additional details about possible contributing factors were released.

Because the collision happened on a major highway, the scene had to be secured while responders checked the wreckage and assessed the injuries. The exact sequence of events was still being reviewed by investigators after the initial emergency response.

Responders treated the scene as a serious two-vehicle collision

Emergency crews were called to the scene shortly after the crash was reported. Initial information from first responders described a head-on collision with two critically injured patients, reflecting how severe the impact appeared at first.

After investigators confirmed the condition of the drivers, the fatality became clear. Mitchell was pronounced dead at the scene, while the driver of the Subaru was transported for treatment. Police identified that driver as being from Columbia City.

The crash drew a large response because of the seriousness of the injuries and the need to manage traffic while medics and investigators worked together. The scene remained active long enough for authorities to gather evidence and assess the vehicles involved.

Highway 30 lanes closed while crews worked the wreckage

Multiple lanes on Highway 30 were closed while the investigation and cleanup were underway. Traffic control was necessary as responders moved through the area and investigators examined the crash site.

Highway closures are common after a serious collision because crews need space to treat victims, document the scene and remove damaged vehicles safely. In this case, the shutdown affected the route south of Scappoose until the roadway could be reopened.

The Oregon Department of Transportation assisted with the response, helping manage conditions along the highway. Their role, along with police and fire crews, was to keep the scene secure and make the roadway safe for passing traffic once the immediate emergency had passed.

Scappoose fire, police and transportation crews assisted Oregon State Police

Oregon State Police were aided at the scene by the Scappoose Fire Department, the Scappoose Police Department and the Oregon Department of Transportation. The coordinated response reflects the number of agencies often needed after a fatal crash on a state highway.

Fire crews can help with vehicle access, patient care and scene safety, while local police assist with traffic and perimeter control. Transportation crews are typically involved when lanes need to be closed or redirected during a roadway investigation.

Officials have not said whether weather, speed or another factor played a role. For now, the public details are limited to the location, the vehicles involved, the number of people hurt and the identification of the woman who died.

Rachel Ann Mitchell identified as the victim in the fatal crash

Authorities identified the woman killed in the collision as 57-year-old Rachel Ann Mitchell. Police did not release her city of residence, and no further personal information was provided.

The crash leaves investigators to piece together how the westbound Subaru crossed into the opposing lanes on Highway 30. That question remains central to the case as officials continue reviewing the fatal collision south of Scappoose.

For now, the known facts are straightforward: a head-on crash on Monday evening left one woman dead, another driver hospitalized with serious injuries and a stretch of highway closed while crews responded. Oregon State Police are leading the investigation.
